Child Care - Information for Parents - How to Apply for Child Care
 
 
Need Help Paying for Child Care?
 
Proofs Needed for Eligibility
 
Before your eligibility forms can be prepared for you to sign, Child Care Services must have proof of your employment, training or schooling status for which you need child care assistance. Proof of income from all sources must also be provided. The following will help you decide what documents to give Child Care Services to prove these things.
 
To Prove Children are Citizens of the United States of America
  • Birth Certificate (Hospital or County), Social Security Card, Passport
 
To Prove Employment
  • Copies of your last 3 paycheck stubs, or
  • Computer printout from your employer, or
  • CCS form filled out by your employer, and
  • Your work schedule (days and times)
 
To Prove Education, Training or Schooling
  • Printout of your current enrollment
  • Copies of transcripts from each place you have attended since you graduated from high school or completed your GED.
  • Printout of enrollment for next semester/quarter if you have completed pre-enrollment
  • Your class schedule
 
To Prove Income
  • Paycheck stubs for employment
  • Printouts from the agency you get child support checks
  • Copy of authorization letter from Social Security or check statement
  • Copy of authorization letter for SSI or check statement
  • Copy of letter showing medical status (such as maternity leave or leave for surgery) and related pay (If none, be sure it states “no pay.”)
  • Statement or printout from anyone else who gives you money, including family members
 
Child Care Services may confirm any income, employment or education/training information you provide. Copies of Social Security cards for family members will also be needed.
 
 
 
 
Your child care assistance cannot begin until
            You have provided all necessary proofs
            You have signed all forms and Child Care Services has received the forms
You have notified Child Care Services of your choice of child care provider
(Must be either a Child Care Services provider or Self-Arranged provider)

If you choose to fax documents to us (806-744-3576),
